In the live Q&A featuring Philippa and another woman (I'm sorry, I can't remember her name; she was from the university), Philippa said the tooth had dropped out in the grave.
So it wasn't knocked out during his lifetime, and hopefully we won't have to deal with a new myth wherein his brother George socked him in the mouth and Richard urged their brother the king to finish off George...or something.

She was Lin Foxhall, head of the Department of Archaeology at Leicester University. In the early articles that followed the discovery of the skeleton, she made the distinction between scoliosis and kyphosis quite clear. I was disappointed that she didn't say anything of the sort in the documentary (she was a walk-on part, so to speak). Maybe she did say it and they edited it out.
